Oral health effects

  • Gum overgrowth from blood-pressure medicines (calcium channel blocker gingival overgrowth)

Calcium channel blockers such as amlodipine and nifedipine can cause fibrous gingival enlargement, mostly in patients with poor plaque control; higher doses and longer use add risk.

Clinical management: Emphasize meticulous plaque control and shorter recall; debride thoroughly; suggest the patient ask the prescriber about an alternative drug if overgrowth progresses; refer for gingivectomy when needed.

  • Gum disease association (periodontitis)

Moderate-to-severe periodontitis is associated with higher odds of hypertension and higher blood pressure readings.

Clinical management: Take blood pressure at visits; manage periodontitis actively; reinforce that periodontal health is part of cardiovascular risk management (treatment effect on BP is inconclusive).

  • Dry mouth from blood-pressure medicines (xerostomia)

Several antihypertensive classes (e.g. diuretics, beta-blockers, ACE inhibitors) are associated with dry mouth.

Clinical management: Review the medication list; saliva substitutes, xylitol and high-fluoride toothpaste; reinforce caries prevention.
