Oral health effects

  • Greater periodontal attachment loss

Osteoporosis in postmenopausal women is associated with more clinical attachment loss and more severe periodontitis. A 2023 meta-analysis found no significant difference in alveolar crestal bone height.

Mechanism: Systemic reduction in bone mineral density affecting the jaws.

Clinical management: Periodontal maintenance; note that antiresorptive therapy used to treat osteoporosis carries a (low) MRONJ risk.

  • Jaw osteonecrosis risk from bone medications (MRONJ)

Bisphosphonates and denosumab used for osteoporosis carry a small risk of exposed, non-healing jawbone, usually after extractions or other bone-invasive procedures (far lower than with cancer-dose regimens).

Clinical management: Record antiresorptive drug, dose and duration; emphasize excellent oral hygiene and regular care to prevent the need for extractions; routine scaling/root planing is not contraindicated; inspect for exposed bone or fistulas and refer promptly.
