Oral health effects
Pregnancy gingivitis
Exaggerated gingival inflammatory response to plaque, peaking in the second/third trimester.
Mechanism: Hormonal (progesterone/estrogen) modulation of the gingival vasculature and immune response.
Clinical management: Reassure that routine dental care is safe; reinforce hygiene; second trimester is ideal for elective care.
Pregnancy tumor (pyogenic granuloma)
Localized, vascular, often interproximal overgrowth that usually resolves postpartum.
Clinical management: Monitor; excise only if symptomatic or persistent.
Tooth erosion from morning sickness (dental erosion)
Frequent vomiting in early pregnancy, and especially hyperemesis gravidarum, exposes teeth to stomach acid and can cause erosion.
Clinical management: Ask about vomiting; advise rinsing with a teaspoon of baking soda in a cup of water (or plain water) after vomiting rather than brushing immediately; use fluoride toothpaste and varnish.
Higher risk of tooth decay (dental caries)
Dental caries is among the most common oral findings in pregnancy, related to frequent snacking, vomiting, and difficulty brushing due to nausea.
Clinical management: Do caries risk assessment; recommend fluoride toothpaste, alcohol-free fluoride rinse and xylitol products; treat active caries during pregnancy (second trimester is ideal); counsel on reducing transmission of caries bacteria to the infant.
