Oral health effects

  • Gum disease (periodontitis) association

RA and periodontitis share inflammatory pathways and frequently co-occur; secondary Sjögren's may add xerostomia.

Mechanism: Shared dysregulated inflammation; P. gingivalis citrullination is implicated in the link.

Clinical management: Periodontal care; manage dry mouth; account for manual dexterity limits in hygiene advice.

  • Jaw joint involvement (temporomandibular joint arthritis)

RA commonly affects the TMJ, causing erosion, flattening and other bony changes, with pain, stiffness and limited opening.

Clinical management: Ask about jaw pain and limited opening; schedule short appointments with breaks and a bite block; avoid prolonged wide opening; refer if function is worsening.

  • Dry mouth from secondary Sjögren's (xerostomia)

Roughly one in five people with RA develop secondary Sjögren's, causing dry mouth and higher caries and candidiasis risk.

Clinical management: Ask about dryness; check salivary flow/pooling; recommend saliva substitutes, high-fluoride toothpaste and shorter caries recalls.
