Oral health effects
Severe mucocutaneous reactions (SJS/TEN)
Carbamazepine carries a notable risk of Stevens-Johnson syndrome / TEN (higher in certain HLA-B*1502 populations), with painful oral ulceration.
Clinical management: Treat suspected SJS/TEN as an emergency; urgent referral.
Xerostomia / dysgeusia
Dry mouth and altered taste reported.
Clinical management: Symptomatic care; reinforce caries prevention.
Aplastic anemia / agranulocytosis (infection, ulceration, bleeding)
Boxed warning: carbamazepine can cause aplastic anemia and agranulocytosis. Mouth ulcers, gingival infection, petechiae or gingival bleeding can be early signs.
Clinical management: Ask about recent blood counts. Unexplained ulcers, gingival bleeding, petechiae or infection with fever warrant urgent referral for a blood count before invasive treatment.
