Doxycycline

Antibiotic (tetracycline)

Brand names: Vibramycin, Oracea

Oral health effects

  • Tooth discoloration (children) / photosensitivity

Tetracyclines bind calcium and cause permanent intrinsic tooth staining if used during tooth development (pregnancy through ~age 8). Sub-antimicrobial doxycycline is also used as a periodontal host-modulating adjunct.

Mechanism: Chelation with calcium during mineralization.

Clinical management: Avoid in pregnancy and young children; counsel on lip/skin photosensitivity.

  • Oral candidiasis (thrush)

Broad-spectrum antibiotics suppress commensal bacteria, allowing Candida overgrowth in the mouth.

Mechanism: Loss of competitive inhibition from normal flora.

Clinical management: Watch for thrush, especially with repeated courses; treat with an antifungal; reinforce hygiene.
