Pramipexole
Antiparkinsonian (dopamine agonist)
Brand names: Mirapex
Oral health effects
Xerostomia (dry mouth)
Reduced salivary flow with elevated caries and discomfort risk.
Clinical management: Hydration, salivary substitutes, high-fluoride caries prevention.
Dizziness on standing (orthostatic hypotension)
Symptomatic orthostatic hypotension is a label warning for dopamine agonists, especially during dose escalation (3% vs 1% on placebo in Parkinson's trials).
Clinical management: Raise the chair slowly at the end of the appointment and have the patient sit upright for a minute or two before standing; steady them as they get up. Ask about dizziness on standing, especially after dose changes or in older patients.
Involuntary movements (dyskinesia)
Dopamine agonists can cause or worsen dyskinesia, including orofacial movements, especially with levodopa (label: 17–18% when added to levodopa (vs 8% with extended-release)).
Clinical management: Expect involuntary jaw, tongue or limb movements that can interrupt instrumentation; use short appointments, bite blocks and good head support, and schedule care when medication is working well (patient-reported 'on' time).
